Membership Application Instructions

Please read carefully before completing the form. When you submit your application, be sure to include:

  1. A copy of your driver's license, I.D. or passport.
  2. A check for $26, of which $25 is the minimum deposit for membership and $1.00 is a membership fee.
  3. Proof of address:  If the address on your driver's license does not match the address which you list on the application, or you are providing us with a passport as a source of identification, please include a copy of a recent utility bill that confirms your present address.
